Supporting adults with age related needs or physical disabilities and their carers. These services include telecare and occupational therapy.
Transfer of Care Teams
Who we support
We help people to move out of hospital back into the community. This includes:

adults aged 65 and over
-
adults aged 18 and over with dementia

adults aged 18 and over with a physical disability
What we do
We focus on a ‘home first’ model. This means we prioritise getting patients home as soon as they are medically fit, with ongoing assessment and support happening in the community.
The Avon Unit at Waterside Care Home in Peckham is part of this service. It provides specialist support for:
- people regaining their independence after a stay in hospital
- people having their needs assessed for their longer-term care or to prevent hospital admission
We work with colleagues across health and social care to ensure a safe transfer of care. We review care and support within 6 weeks and tailor it to meet long term needs.
