Supporting adults with age related needs or physical disabilities and their carers. These services include telecare and occupational therapy.

Who we support

We work across adult and children's social care, supporting:

  • adults with a physical disability aged 65 and over
  • older adults with a physical disability or dementia aged 18 and over
  • children with a physical disability aged 0 to 17
  • carers 

What we do  

Preventing and reducing the need for care and support is a key focus for our team.  We assess people’s needs by looking at the whole picture and focusing on their strengths. We often suggest assistive equipment to help residents do everyday tasks more independently.

We may also recommend minor and major adaptations to the home. We work closely with housing colleagues and providers to help ensure that residents can stay at home for longer. Where adaptations are not suitable or possible and the resident is willing to move, we may provide a rehousing report to support them to do so.    

We support carers to carry out safer, more efficient moving and handling. We recommend equipment and demonstrate new ways to carry out tasks. 

We work closely with professionals across health and social care, the community and the voluntary sector. We provide advice and signposting to other services to ensure that residents have the right level of care and support.
