Supporting adults with age related needs or physical disabilities and their carers. These services include telecare and occupational therapy.

What we do
Our team is made up of social workers and social work assistant practitioners. We focus on supporting residents with more complex needs. This may involve supporting them through major life transitions, like moving into supported living, residential, or nursing care placements.
We complete more long-term, complex work on safeguarding cases and mental capacity assessments. We work with residents and their families to ensure decisions are made in their best interests.
We hold legal cases and do Court of Protection work, helping keep vulnerable residents safe in their communities and safeguarding residents from harm and the risk of abuse. We also complete carers' assessments to support unpaid carers.
