Older people and physical disabilities service (OPPD)

Supporting adults with age related needs or physical disabilities and their carers. These services include telecare and occupational therapy.

Assessment and Intake Team

Who we support 

We work with: 

  • adults aged 65 and over

  • adults aged 18 and over with dementia

  • adults aged 18 and over with a physical disability 

  • carers

  • individuals experiencing, or at risk of, abuse or neglect 

What we do 

Our team is made up of social workers and social work assistant practitioners. We complete strengths-based Care Act assessments to determine eligibility for care and support.

We also complete reviews and reassessments of people in residential care who have experienced a change in need. 

We provide commissioned support through:

  • domiciliary homecare support
  • day-care
  • respite care
  • residential and nursing care placements

We help carers by talking with them about their strengths (carers assessments), and provide safeguarding support for those experiencing, or at risk of, abuse or neglect. 

We focus on short-term interventions.  Where ongoing support is needed, we transfer the case to the case management team.
