Older people and physical disabilities service (OPPD)

Supporting adults with age related needs or physical disabilities and their carers. These services include telecare and occupational therapy.

Intermediate Care Southwark (ICS)

Who we support

We work with: 

  • adults aged 65 and over

  • adults aged 18 and over with dementia

  • adults aged 18 and over with a physical disability 

  • carers

  • individuals experiencing, or at risk of, abuse or neglect 

What we do

ICS are a multi-disciplinary team of:

  • physiotherapists
  • occupational therapists
  • social workers
  • assistant practitioners
  • nurses
  • pharmacists
  • rehabilitation support workers

We work with a dedicated reablement care provider. We also work closely with partner agencies and other teams in OPPD.    

We prevent unnecessary hospital admission or readmission by providing urgent interventions at home to ensure residents remain as independent as possible.

Our team of enabling care staff provide exercise programmes, practice daily living skills and prescribe equipment to help promote independence. We also offer a period of rehabilitation and reablement to help maintain or increase residents’ independence at home. 

Where there may be a need for long-term care and support after reablement, we complete Care Act assessments. We plan care by looking at the whole picture, working with them and their carers and focusing on their strengths.
