These teams work together to support disabled people from childhood through to adulthood.

Support for adults with mental health needs. This includes reablement, recovery from substance misuse and support for those detained in hospital.

Supporting people with age related needs and physical disabilities and their carers. These services include telecare and occupational therapy.
