Who we support 

We work with: 

  • adults aged 65 and over

  • adults aged 18 and over with dementia

  • adults aged 18 and over with a physical disability 

  • carers

  • individuals experiencing, or at risk of, abuse or neglect 

What we do 

Our team is made up of customer support officers, social workers and occupational therapists. 

We are the first point of contact for the older people and physical disabilities service (OPPD). We offer information and advice, complete initial screenings of residents with care and support needs, then connect them to the right teams to get the support they need.  

The contact team’s occupational therapists prioritise early intervention and prevention. They provide interventions, including equipment and minor adaptations to promote independence and reduce the need for care and support. All staff are trusted assessors. 

The team provides safeguarding support for those at risk of or experiencing abuse. They ensure safeguarding enquiries are transferred to the longer-term teams in OPPD.  

Our focus is on promoting independence and resilience, using a strengths-based, community-focused approach. We connect residents to voluntary and community services (VCS) where appropriate, to make sure they get the right level of support.
