Who we support
We work with adults with mental health needs, living in a range of mental health settings.
Our goal is to help them improve their wellbeing and make informed, meaningful decisions about their lives.
What we do
Our team is made up of social workers and occupational therapists. We do assessments, reviews, and reassessments for people living in residential, nursing, or forensic mental health placements.
At the start of our involvement, a MOST worker will focus on understanding the individual’s values, goals, and daily living skills.
We work with adults to produce a support plan, tailored to improving quality of life in line with their own preferences. This may include supporting a transition to more independent living.
Where appropriate, we arrange care packages to improve support in supported housing environments. For example, help with daily tasks like cooking, shopping, or accessing community activities.
