Mental Health Reablement team
Who we support
We help adults aged 18 to 64 where mental illness is the primary need.
What we do
We are usually the first team that responds to residents experiencing mental health difficulties. Our team is made up of social workers and occupational therapists.
We work closely with voluntary, charity and community organisations like Southwark Wellbeing Hub, who also refer people to us.
Our aim is to prevent, reduce, or delay the need for ongoing social care. We offer residents up to 10 weeks of focused support to help them work towards their personal goals.
Staff are very 'hands on' and coach residents to maximise independence in managing their home environment. We may provide support for the service user to engage with community activities, or help them manage daily tasks like shopping.
If we assess someone as needing continued support, we arrange appropriate services and transition them to a suitable long-term team.
