| Health checks and social prescribing | Your GP should record your carer status on your patient record and offer you a health check if you are between 40 and 74. They can also refer you to a ‘social prescriber’ who can help you access other support. |
| Mobilise library | The library is filled with support guides and carers’ practical tips on how to keep active and take care of yourself. |
