How to apply

If you're a parent or about to become one and you live in Southwark, you can apply for a free trial pack by filling out an application form and providing scanned copies of the following:

  1. Proof of address: such as Council Tax statement, tenancy agreement, or a recent utility bill
  2. Proof of pregnancy or birth, such as MatB1 Form (both sides), NHS prescription Charge Card, baby's Birth Certificate, an official document with your name and the baby's date of birth or expected due date.
