Older people and physical disabilities (OPPD)

If you're 65 or older, or an adult with dementia or physical disabilities, our older people and physical disabilities team may be able to support you.

Make a referral to the OPPD team

Refer yourself, a friend or family member

To request daily care support for yourself or someone else complete our online referral form.

Call us 020 7525 3324 if you:

  • need urgent support (within 48 hours)
  • would like advice
  • would like to be connected to a local community service
  • are unable to complete a referral online

Find out what to do if an adult is experiencing, or at risk of, neglect or abuse

In an emergency always call 999.

Make a professional referral

Referrals for care, occupational therapy, telecare or sensory support

If you're a professional and would like to refer someone for support complete our online referral form.

Urgent referrals

Our response to referrals is based on their level of urgency and risk.

If you're concerned that the person you're referring would be at significant risk if not supported within:

  • 48 hours, call the @home service on 020 3049 5751 for an urgent community response

  • 14 days, select the 'priority referral' on the referral form.

Unpaid carer support

If you would like to request support for someone who is providing an unpaid carer role to a friend or family member, please download and complete the carer referral form for professionals.

Make a referral for carer support

If you're providing a caring role for a family member or friend you may be able to get support. You can get advice and support from Southwark Carers and Mobilise.

You can also request a carers assessment. It is not a test of your caring abilities. It's for us to assess what support, if any, could help you in your caring role.

This support could include:
• education and training
• practical help in the home
• support for your mental and physical wellbeing

You do not need to be living with the person you look after, or be providing full-time care.

You can also refer someone else for a carer's assessment.
