Our adult social care teams

Find out about the different teams and the services they offer. 

Older people and physical disabilities (OPPD)

If you're 65 or older, or an adult with dementia or physical disabilities, our older people and physical disabilities team may be able to support you.

Young adults with disabilities

If you have a disability and are under 25 (or their carer) our all age disability team may be able to support you.

Learning disabilities

If you have a learning disability you may be able to get support from our community learning disabilities team.

Southwark Wellbeing Hub and other mental health services

If you're struggling with your mental health the Southwark Wellbeing Hub helps you get the right support.
