All-Age Disabilities (AAD)
Who we support
We are dedicated to improving the life outcomes and promoting the independence of disabled children and young people aged 0 to 25.
What we do
We offer a seamless, integrated social care pathway from early childhood through adolescence and into adulthood.
This includes:
- assessments
- complex case management
- safeguarding
- support planning reviews
- practical support for families
Our service is unique in working across the legislative frameworks of:
- the Children and Families Act 2014
- the Care Act 2014
This gives our social workers the opportunity to develop broad expertise across all age groups.
We also work with looked after children and carry out court work and care proceedings.
Our practice is informed by looking at relationships and the broader context of people’s lives.
The service consists of 4 teams made up of social workers and assistant practitioners. A duty team manages new referrals and urgent situations.
With manageable caseloads, staff can focus on delivering high quality support.
We offer a supportive environment that encourages continuous professional development through peer supervision, 'lunch and learn' sessions, and collaborative case discussions with external experts.
