These teams work together to support disabled people from childhood through to adulthood.
Learning Disability service (LD)
Who we support
We support adults aged 25 and over with learning disabilities or autism who are eligible for support under the Care Act.
For example, we support individuals who are:
- living with carers
- navigating the criminal justice system.
- living with dual mental health diagnoses
- parenting with a disability
The diversity of needs creates a rich environment for social work practice.
What we do
We work in close partnership with the All-Age Disabilities team to ensure smooth transitions into adult services at age 25. We also accept referrals from across the wider community.
Our service users benefit from support through strong multidisciplinary teamwork with:
- specialist learning disabilities health services
- psychologists
- psychiatrists
- nurses
- occupational therapists
- physiotherapists
- speech and language therapists
In this service you’ll find a supportive management team committed to your professional growth. We offer a range of development opportunities designed to build expertise and improve outcomes for our residents.
Peer supervision and informal learning are embedded in our culture. Social workers lead regular ‘lunch and learn’ sessions, and external professionals facilitate case discussions.
