London Care Record privacy notice

The London Care Record (LCR) is a secure electronic system that allows important information about your health and care to be shared among different healthcare organisations and social care professionals. It helps them quickly and securely access the information they need to make the best decisions about your care.

The London Care Record contains some or all of the following information about you:

  • name, date of birth, gender, address, and contact information, along with your NHS number, which helps identify you
  • GP details (doctor), appointments, and visits to hospitals
  • current medical conditions, medications, and medical history
  • results of tests and scans, as well as records of any hospital referrals, admissions, discharges, and clinics you have attended
  • documents such as health and social care plans, clinic letters and discharge summaries

Only health and social care professionals directly involved in assessing or providing your care and who have a lawful reason to access your records can do so through the London Care Record. This ensures that your information remains confidential and is only shared with those who need it to help you.

If you do not wish for your records to be shared through the London Care Record, you have the option to opt out by completing the Individual Rights form.
