All-Age Disability (0 to 25 years) Team
If you have a disability and are under 25, our all-age disability team may be able to support you.
The All-Age Disability Team (AAD) is a team of social workers and assistant practitioners who arrange support for children and young people with disabilities aged 0 to 25. It is a specialist service for those with severe and profound needs.
Any support offered is subject to an assessment and regular reviews.
Who the AAD service is for
The AAD service offers assessments for individuals with:
- severe and profound, long-lasting or permanent disabilities
- life-limiting or life-threatening conditions
A disability may be classed as severe and profound if a child or young person needs significant support from another person or equipment to carry out basic daily functions, like:
- going to the toilet
- washing
- sleeping
- eating
- participating in activities
You can self-refer to the service or be referred by another agency, like your child’s school or a health professional.
If eligible for the service, a care plan will be agreed for your child after an assessment. This plan will say what everyone around your child will do to help meet their needs. It may include support in the form of a care package of services for your child.
Any support offered will focus on your child's individual needs and family circumstances. The needs of parents and carers are considered as part of the assessment, but if you'd like to have your needs considered separately, you can request a carer's assessment. If your child is under 18, you can complete a self-assessment.
