How to appeal a decision

Stage 1

If you wish to appeal either an eligibility or mode decision, complete the relevant Stage 1 appeal request online or download a copy of the eligibility appeal form (docx, 50kb). You must submit the form within 20 working days of the decision you wish to appeal.  

If you're appealing because of medical issues, we usually only consider those issues if they're supported by a recent letter from a medical consultant. We won't normally consider letters of support from GPs.

If you feel that the type of travel assistance offered isn't sufficient to meet the child’s / young person’s needs, request an appeal request mode online or download a copy of the appeal request form (docx, 49kb).

Stage 2

If you wish to appeal a Stage 1 decision, complete the Stage 2 appeal form online or by downloading a copy Stage 2 appeal form (doc, 82kb) within 20 working days of the Stage 1 decision you wish to appeal.
