Integrated Care Southwark, which incorporates rehabilitation services and intermediate care teams, is one of the teams that has been rated ‘outstanding’ by the Care Quality Commission (CQC).

The rating comes as part of a recent inspection of Guy’s and St Thomas’ NHS Foundation Trust as a whole.

Integrated Care Southwark was established to create more joined-up services between health and social care services for local residents and is a partnership between Guy’s and St Thomas’ community services and Southwark Council.

The service is centered on early interventions to avoid hospital admissions, provide support for people coming out of hospital to get their independence back and support those living with long term health conditions.

Inspectors praised how “staff demonstrated outstanding compassionate care and emotional support to patients and those close to them.” It was also noted that staff involved patients and their families in decisions about their care and treatment observing that “staff had an inclusive approach to enhance patient experience placing great emphasis on their emotional wellbeing.”

The report also highlighted that there was “a strong and visible culture of person-centred care which in turn meant staff provided outstanding levels of compassionate care to patients.”

As well as the compassionate care staff provide, inspectors also noted the common purpose among staff and managers to drive improvement through innovation. The leaders, managers and staff had a strong focus on ensuring patients had access to the right care at the right time, to prevent avoidable admissions to hospital.

Cllr Jasmine Ali, cabinet member for children, schools and adult care, said: “This report highlights what can be achieved by health and social care services working together to provide a holistic service for our residents. I am incredibly pleased that the hard-work and dedication of council and NHS staff has been recognised in this report.

“Southwark Council is committed to working alongside our partners to deliver services that are having a real impact on our residents’ lives.”
