Register a death
Learn how to register a death. Our bereavement support service.
Register a death in person within 5 days unless a coroner is conducting an investigation. You can do this if:
- you're a relative of the person who has died
- you were present at the death (and you're an adult)
- you're instructing the funeral director
- you know the circumstances of the death and you run or manage the place where the person who died lived, such as a care home
To register a death, you'll need:
- a death certificate signed by a certified medical practitioner; email it to mccd@southwark.gov.uk before your appointment
- to know when and where the death occurred
- the full name and usual address of the person who died
- their maiden name (in the case of a married woman)
- their date and place of birth
- their occupation
- their NHS number
Book an appointment to register a death.
There's no charge for registering a death. Death certificates cost £12.50 each and can be pre ordered at the time of booking appointment to register the death or at the time of registration.
