NHS Health Check

The NHS Health Check is a free health check-up helping you prevent the onset of diabetes, heart disease, kidney disease, stroke and certain types of dementia.

If you're aged 40 to 74 years old without a pre-existing condition, you're eligible for a free health check every five years.

If you haven't received an invitation letter, contact your local GP. You can also have a health check at certain pharmacies (pdf, 3.2mb).

Read the leaflets for more information about health checks:

Living a healthy lifestyle can lower your chances of developing a long-term health condition, eg maintaining a healthy weight, eating healthily, being active, stop smoking and drinking less.

Digital Health Check

If you've received an invitation via a SMS Text message or letter to complete an online Southwark Digital Health Check and would like more information, see our FAQs (pdf, 80kb).
