Joint Health and Wellbeing Strategy

The Southwark Joint Health and Wellbeing Strategy (PDF, 1.3mb) was adopted by the Health and Wellbeing Board in 2015 and sets out our partnership vision to improve health and reduce health inequalities in the borough. The Joint Health and Wellbeing Strategy provides the overarching framework for shaping major policies and actions in Southwark.

The six key objectives in Southwark’s Joint Health and Wellbeing Strategy are:

  • giving every child and young person the best start in life
  • addressing the wider socio economic and environmental determinants of health: maximising opportunities for economic wellbeing, creating jobs & apprenticeships, making homes warm, dry and safe and creating healthier places to live in
  • preventing ill health by promoting and supporting positive lifestyle changes and responsibility for own health and improving people’s wellbeing, resilience and connectedness
  • helping people with existing long term health conditions to remain healthier and live longer lives by improving detection & management of health conditions including self management and support
  • tackling neglect and vulnerabilities by supporting vulnerable children and young people and ensuring positive transition, ensuring choice and control for people with disabilities and supporting independent living for older people in an age friendly borough
  • supporting integration for better health and wellbeing outcomes by integrating health and social care that is personalised and coordinated in collaboration with individuals, carers & families and by shifting away from over reliance on acute care towards primary care and self care

We're developing a series of thematic strategies and action plans for each of the priority areas which will underpin the overarching Joint Health and Wellbeing Strategy. The Health and Wellbeing Board receives regular reports on progress and performance of these priority areas. Further information is available under our strategies.
