This form is for schools to apply for short-term alternative provision (AP) for pupils who cannot attend school for medical needs (15 days or more) or other exceptional reasons.
As the local authority, we have this duty under section 19 of the Education Act. It is intended as a temporary arrangement. Pupils are expected to return to school as soon as they are well enough or it is otherwise suitable.
For all medical applications, a separate medical evidence form must be completed.
Alternative provision panel meetings are held on Mondays during term time. Completed referral forms must be received by 5pm on the Monday before the next panel meeting.
