Family Early Help Specialist Community Public Health Nurse

Health issues often play a significant role in a family’s difficulties and can contribute to or complicate other issues, including family relationships, financial wellbeing and school attendance.

To reduce the impact of these factors, all families who receive a Family Early Help service can access assessment and intervention from our Specialist Community Public Health Nurse. This integrated  health practitioner offers bespoke outreach family health assessments to identify unmet health needs in any member of the household to ensure that the appropriate support and treatment is secured. This may be through direct intervention from the Family Early Help Specialist Nurse or by supporting families to engage with a range of health services that they previously were unaware or may have struggled to access. In addition, part of the duties of the Specialist Nurse is to fulfil the role of a school nurse where children or young people are out of education for a significant period and so might not have received all of these universal health services.
