Serious case reviews

When a child dies and abuse or neglect is known or suspected to be a factor in the death, the Southwark safeguarding children board (SSCB) should always conduct a serious case review (SCR) into the involvement of organisations and professionals in the lives of the child and family.

In addition the SSCB should consider whether to conduct an SCR whenever a child has been seriously harmed and the case gives rise to concerns about the way in which local professionals and services worked together to safeguard and promote the welfare of children.
Southwark Safeguarding board has completed the following three SCRs:

  • Child G - a baby who died in 2007   
  • Child 'Jack' - a baby who died in 2008
  • Child I - a baby who was seriously injured in 2009

In all three cases the police investigations into the incidents and subsequent court cases have delayed the publication of the executive summary reports.
All of the police investigations have now concluded and the executive summary reports are now formally published. They have been anonymised to avoid identification of any individuals concerned.

Anyone accessing these reports should be aware that this is sensitive information relating to real children and their families. The summaries should be read with respect and destroyed when no longer needed
