Our revised approach to the common assessment tries to make families at the centre of what we do a reality. It is a simpler and clearer approach to assessment and the delivery of help and to ensuring the delivery of better outcomes for Southwark's children and young people. The new approach highlights the significance of the relationships families have with the practitioners working with them, and the need to provide early help with (not to) families. We have developed revised guidance on the CAF process. We have also developed local versions of the various parts of the Southwark CAF toolkit:

The Southwark CAF assessment record itself, to record the information and analysis of the strengths and needs of a child, young person and family
- The delivery plan and review record, the help the Team Around the Child (TAC) deliver a single plan of help for families, and to monitor and review progress
