Moving from children's to adult services can be challenging for young people with disabilities or special educational needs.

'Time to Think! for young people' takes young people through the main choices on offer. It asks key questions to help them decide what they would want for the future. A symbol tool called 'Supporting Time To Think', has also been created. The purpose of this support tool is to aid young people who require additional support with communication.
'Time to Think! for parents' looks at young people's choices from the perspective of the parent/carer. It focuses on supporting the young person's independence. The two guides have also been put together to help young people and their parents prepare for the Year 9 annual review, which is also the transition review.
'Moving On' is an information pack on transition to adulthood. It has detailed information on the different options after school, the transition process into adult services, information on transport, leisure and community services, as well as information on benefits and financial support.
