Information Sharing and Assessment

Common tools and processes are essential if we are to deliver improved outcomes for children, young people and their families in Southwark. We are developing a variety of common tools and processes to equip professionals with the skills and knowledge they need to ensure improved and more integrated service delivery.

Key areas of this work include

Under the remit of the common tools and processes work programme, the areas of development in progress are as follows

  • Information sharing
  • Implementation of the Common Assessment Framework (CAF) 
  • Development around information, technology and communications
  • Directory of information on services and provision within the borough

The Integrated working manual (pdf 1.41mb) brings together the procedures, forms and tools that have been developed in conjunction with Southwark practitioners and managers on all of these areas of work, it is a key tool for supporting integrated working processes.

Common Assessment Framework

The common assessment framework (CAF) is a key component of the Every Child Matters change programme, and a tool to encourage responsive and earlier assistance to our children, young people, and families requiring additional help.

The CAF will encourage better communication between practitioners through the use of a common language and shared understanding of different levels of need. It provides a broad and consistent basis for gathering and analysing information within a universal setting (such as a school), as well as providing a foundation of information which can be built on by other services and professionals delivering more targeted and specialist support.
