Our Partnership with Southwark Primary Care TrustSouthwark social care services has joined in partnership with Southwark Primary Care Trust (PCT) - our local NHS trust, to improve the health and social care of Southwark residents.
What does this mean for Southwark residents?
Southwark Health and Social Care aims to meet your health and social care needs together by joining up many of their services so that you do not have to go to many different teams for help.
Most teams are already integrated and are enabling us to deliver your service more efficiently. For example, our work will mean that one initial assessment of your needs allows you to access a variety of health and social services quickly and easily.
Why integrated services are better

- Reducing the maximum wait in accident and emergency
- Reducing the number of hospital beds occupied by people ready for discharge
- Improving mental health services, particularly in areas of untreated psychosis, crisis, outreach and improving service provision for carers and older people with mental health problems
- Improving services for older people
- Reducing teenage pregnancy
- Tackling drug misuse
- Improving services for children - child protection, children’s mental health services, our work with children in care and our implementation of the children’s national service framework
Southwark is in a unique position as our strategic director of health and community services, Susanna White is also the chief executive of the PCT. This demonstrates our commitment to delivering integrated services to improve the health and wellbeing of Southwark.
