Outreach ServiceOur outreach service supports adults with physical disabilities to live a healthy and fulfilling life. We can help service users to build up all aspects of their lives in order to decrease social isolation and increase their overall quality of life.
Who is the service for?
The outreach service is for adults over 18 years of age who have a physical disability and who live in Southwark.
How can we help?
We can help, or give information about
- Overcoming barriers experienced in accessing employment, education and leisure opportunities
- Personal development and self-advocacy
- Education with regard to opportunities available in the wider community
- Developing and achieving personal goals through a self-directed service plan
- Achieving and maintaining a healthy leisure lifestyle
Is there a charge for the service?
There is no cost involved for this service and we are currently accepting all new referrals.
How to use this service
To use the service you will need to be referred by a social worker.
