Physical Disabilities If you are a physically disabled person aged 18 to 65 years old, or their carer, we provide a care package based around your individual needs. We aim to help you live independently in your own home, or if your needs cannot be met at home, to provide you with high quality residential or nursing care.
How we can help
Once you are referred to our service, a member of the team will carry out a needs assessment with you to find out what type of help you may need. In addition to general day to day care support, we can arrange other services for you, which may include
The care we provide is kept under continuous review. All the health and social care workers in the team are professionally qualified and experienced in working with people with physical disabilities.
Visual impairment rehabilitation
Our visual impairment rehabilitation officers work with those in Southwark who are visually impaired. After receiving clients' registration forms, usually from ophthalmologist or hospitals, we write to the client asking if they would like an assessment provided by one of the rehabilitation officers. Once the assessment is complete we can offer
