Accident Reporting

The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR) requires all employers to provide and/or make arrangements to keep a written record (accident book), of all injuries, accidents, dangerous occurrences or cases of disease at the workplace or at the usual place of business of the employer, for at least 3 years from the date on which it was made.

The written record may either be kept in an prescribed form (Accident Book P10015, obtainable from HSE Books) or must contain the following information:

  • Date of the accident
  • Name of the person injured
  • Sex of the person injured
  • Age of the person injured
  • Occupation of the person injured
  • Nature of the injury
  • Place where the injury was sustained
  • A brief description of the circumstances

For prescribed situations these accidents are notifiable to the local health and safety enforcing authority. In general, an accident becomes notifiable where an employee is absent for 3 days or more, or where he/she is hospitalised for 24 hours or more as a result of injury at work.

Please note that there are other instances where injuries, diseases and dangerous occurrences become notifiable and within certain time limits. In cases of uncertainty you are advised to report in the first instance immediately by telephone. Failure to report or late reporting is an offence.

Where there is a notifiable accident i.e. death or major injury to any person the responsible person shall immediately notify the enforcing authority by the quickest practicable means - normally either by telephone or fax using the approved form F2508, available on line from the RIDDOR website.
